Requesters note: the requester acknowledges that a similar request for the same document was made to the clerk of the court whom [REDACTED]. The office of the Judiciary Courts of the State of Florida is in the same building as the clerk of the court [REDACTED]. The office of the Judiciary Courts of the State of Florida is in ste 106 and the managers name is David Fina whose assistant is Millie Scheinder. This new request gives the contact info for Fina and Schiender whom are directly responsible for supplying the correct IRS W-9 form. The requester has the DUNNS number, Cage Number and UEI number but needs the IRS W-9 form to complete the investigation. Please provide the electronic copy of IRS form W-9 for the “Judiciary Courts of the State of Florida”. I am hoping to find out the Employer Identification Number [EIN] of this public entity.

Although I’m requesting the EIN for this particular branch of the “Judiciary Courts of the State of Florida”, I believe that there are 484 Branches and one Subsidiary operating under the same name (“Judiciary Courts of the State of Florida”) ... which may all be under one umbrella, and would mean there is only one EIN for the entire system. If this is the case, then please provide me with that EIN.

It’s the requester’s responsibility to write the request in a manner that clearly specifies a record that is being requested, and to what government entity it should be sent.

Please specify the exact type of records you’d like to receive, rather than any “possible” or “potential” info that could be searched.
